Serious Incident Reporting Requirements

NHS Standard Contract holders must adhere to the Serious incident reporting requirements as laid out in the service conditions (SC33) of the NHS Standard Contract, reporting incidents to the Care Quality Commission (CQC). The revised Serious Incident Framework outlines the process for responding to serious incidents in NHS-funded care. The Framework can aid organisations to identify and learn from what went wrong and determine how to minimise the risk of another occurrence.  An example of an incident report form is available on page 75 of the Framework.
